Food Safety and Nutrition Survey (FSANS)
The Food Safety and Nutrition Survey (FSANS) is FDA's national probability consumer survey designed to assess consumers' awareness, knowledge, understanding, and self-reported behaviors relating to a variety of food safety and nutrition-related topics. The survey findings are intended to help FDA make better-informed regulatory, education, and other risk-management decisions aimed at promoting and protecting public health.
What is the Food Safety and Nutrition Survey (FSANS)?
The FSANS is the primary source of information in the nation about food safety and nutrition knowledge, attitudes, and practices. This survey helps the U.S. Food and Drug Administration develop educational materials and programs to better serve the American people.

Your address was randomly selected from a list of all the residential mailing addresses in the nation. It was selected using scientific sampling methods to represent households in the United States.
Why should I take part in this survey? Do I have to do this?
The chances of being selected for this survey are small. Once selected, you represent thousands of other households like yours. In other words, because we cannot include everyone in the survey, your answers represent the answers of other households like yours. Your answers and opinions are very important to the success of this survey.
Your participation is voluntary. You may choose not to answer any or all questions in this survey. There are no penalties should you choose not to participate in the survey.
How will the information I provide be kept private?
Your answers will be kept private under the Privacy Act. Your answers cannot be linked to any information that could identify you or your household and will be kept secure to the extent permitted by law.

The survey will take 15-20 minutes for you to respond.
