As Chief Medical Officer for a four hospital health system and Chair of our Patient Safety Improvement Committee, I have become familiar with best practices that are advocated for the minimization of adverse events. The Institute of Medicine report reminded us of the undesirable and unintentional consequences of inadvertent clinical occurrences. Bar code reconciliation has shown significant promise (with respect to evidence based medicine) for the diminution of administration related medication errors. A standardized system of bar coding of medications would greatly increase the feasibility and practicality of such an intervention. Please consider supporting mandatory bar code labeling for all human pharmaceuticals. Thank you.
