On my draft form the words all run together which makes the form difficult to follow. I feel the blocks for Lot Number, Route and Site are not large enough and the people processing this form (myself)will have trouble reading the information. I feel lines are much better than blocks for reporting information. Blocks are too limiting. Block #8 is a complete mess. I feel this form is a mess and hard to read and will aggravate those who are trying to report adverse reactions.
