What Information Is Required? (cont)

A statement that the information submitted is true and accurate and that the individual submitting the form (if not the owner, operator, or agent in charge) is authorized to do so.

The submitter, if not the owner, operator, or agent in charge, also must provide the name and contact information of the individual who authorized submission of the form
