Application Checklist

The Class of 2014 application period is closed.

Please gather the following items BEFORE applying for the Commissioner’s Fellowship Program.  A link to the online application will be placed on this website tentatively next spring. 

Late and/or incomplete applications will not be considered.

Application period for the CFP Class of 2014: 

  • Applications were accepted from April 16, 2014 through May 26, 2014 5 p.m. EST.  Letters of recommendation were due June 4th by 5 p.m. EST.

  • July through August – Applicants Notified

  • October – Program Start Date

Green Card number if you are not a U.S. Citizen.

Verification of foreign degree if your degree is completed outside of U.S. (Please visit here for more guidance.)  An electronic copy (PDF) is required.

Electronic copy of your Resume/CV in PDF format only.

Three references and their contact information.  These references will receive email notifications requesting them to submit their letters electronically and they must submit their letters as PDFs.  A personal e-mail address is required for each reference.

Personal statement in PDF format which outlines both your educational and professional experience, as well as what has driven you to apply to the FDA Commissioner’s Fellowship Program. Address what you would bring to the FDA and how you believe this Fellowship will help you reach your short and long term career goals. Please limit your statement to one, single-spaced page, using a 12-point font.

Electronic copy of your transcripts in PDF format.  Unofficial transcripts are acceptable. Only one file can be uploaded per application so either submit a transcript for the most relevant degree, or combine all transcripts into one file. Go to the Commissioner's Fellowship Program website. Look at the proposed projects and select up to five. Write down the Preceptor names so that you can provide them during the application process. If a project has multiple Preceptors, you only need to list one of their names.

Important Notes:

  • Do not include your Social Security Number on any of your application materials.
  • Files over 20MB can not be uploaded into the application.  We only accept PDF files.
  • The Commissioner’s Fellowship Program is not responsible for technical issues unrelated to the application database that may result in a late or incomplete application. 
  • When an applicant submits a complete application the system will provide a confirmation number and will also automatically send three reference request emails to the references and the applicant will be copied on these messages.  Applicants should retain the confirmation number and reference letter request emails.  Applicants are responsible for ensuring that their references submit their letters on time.

Privacy Act Notice: This notice is provided pursuant to the Privacy Act of 1974, 5 U.S.C. 552a. The collection of this information is authorized by 5 U.S.C. 301, 1302, 3301, 3302, 4101-4121 (Chapter 41 of Title 5), 42 U.S.C. 209(g) and Executive Order 10577. FDA will use the information you submit to process your Commissioner’s Fellowship Program (CFP) application, determine suitability for employment, and to select candidates. Your application for a CFP appointment is voluntary. Should you choose to apply, it is mandatory that you provide the requested information. Failure to supply the information may prevent FDA from considering your application.

FDA may disclose information to recipients outside the FDA and the Department of Health and Human Services (HHS) including: the Office of Personnel Management, the Equal Employment Opportunity Commission and similar federal agencies with authority over labor and employment matters; Members of Congress; the courts and the Department of Justice in the context of litigation and requests for legal advice; the appropriate investigating and/or enforcing agencies when the records indicate a violation or potential violation of law; federal, state or local agencies maintaining civil, criminal or other relevant enforcement records or other pertinent records, such as current licenses; a federal agency, in response to its request, in connection with the hiring or retention of an employee, the issuance of a security clearance, the reporting of an investigation of an employee, the letting of a contract, or the issuance of a license, grant, or other benefit by the requesting agency; other federal agencies in response to subpoenas issued by such agencies; contract employees whose duties require the records; and, the Department of Homeland Security and other federal agencies and contractors in order to respond to system breaches.
