1. PURPOSE
This Guide establishes procedures for obtaining door title signs for corridors and inner offices at Headquarters.
2. APPLICABILITY
This Guide applies to the following Headquarters buildings: Parklawn, Park, and FB-8.
All other buildings have their own procedures adapted to the requirements of their particular building managers.
3. RESPONSIBILITY
The Division of Management Services, Administrative Services Branch is responsible for obtaining door title signs for Headquarters offices.
4. DESCRIPTION
The basic design is a card 6 inches by 8 inches with white lettering on a black background at the Parklawn and Park Buildings, and white lettering on a blue background at FB-8. The room numbers appear in 1 inch lettering, major organization (i.e., FDA) in 1/2 inch lettering, and subordinate titles in 1/4 inch lettering. The subordinate titles are limited to a maximum of six lines and a maximum of 35 characters per line, including periods, commas, and spaces. If a door is not to be used as an entrance, the room number and FDA is printed at the top and "Enter Room " with an arrow indicating the proper direction is to be printed on the bottom.
5. SUBMISSION OF REQUESTS
A. Parklawn and Park Buildings. Offices should submit two copies of Form PHS-6028, "Request for Door Title Cards and Holders" to the Services Unit (HFA-227).
B. FB-8. Offices should submit two copies of GSA Optional Form 12, "Request for Door Title Cards and Holders" to the Metropolitan Office Services Unit (HFA-216).
